Personcentrerad vård är kärnan i reumatologisk omvårdnad

För personer med kronisk inflammatorisk artrit och biologisk läkemedelsbehandling är livet ofta kantat av bedömningar, administration, behandling och uppföljningar av både sjukdom och biologiskt läkemedel. Ingrid Larsson har i en doktorsavhandling vid Hälsohögskolan i Jönköping visat att sjuksköterskan har en viktig roll för dessa patienter och att en personcentrerad vård stärker patienters trygghet, förtrogenhet, delaktighet och oberoende.

Syftet med avhandlingen var att utforska och utvärdera den reumatologiska omvårdnaden utifrån ett personcentrerat perspektiv för patienter med biologisk läkemedelsbehandling.

En personcentrerad vård är respektfull, individanpassad och personstärkande samt har en helhetsyn där patienter ses som unika personer. I en personcentrerad vård sker ett möte och en förhandling mellan två experter: patienten som expert på sin sjukdom/ohälsa och sjuksköterskan som expert i sin profession.

– Patienters trygghet och känsla av förtrogenhet samverkar och ökar med en personcentrerad vård, vilket även patienters delaktighet och oberoendet gör. Detta är en pågående process där personcentrerad vård är kärnan som bidrar till en positiv samverkan, säger Ingrid Larsson.

Ingrid Larssons avhandling baseras på fyra studier med patienter som har kronisk inflammatorisk artrit (t.ex. reumatoid artrit och spondylartit) och som är behandlade med biologiska läkemedel. Resultaten visade på att patienter med intravenös läkemedelsbehandling uppfattade en regelbunden kontakt med sjuksköterskan som trygg, delaktig och energigivande. Patienter som själva administrerade sitt biologiska läkemedel med subkutana injektioner fick kämpa för att bli delaktiga och oberoende i livet.

– För att ytterligare förbättra omhändertagandet av dessa patienter utformades en sjuksköterskeledd mottagning, baserad på personcentrerad vård vars syfte var att ersätta vartannat läkarbesök i uppföljningen av den biologiska läkemedelsbehandlingen. En sjuksköterska och en läkare har olika perspektiv och kompletterar varandra. När patienten får möjlighet att träffa flera professioner regelbundet blir vården mer komplett, förklarar Ingrid Larsson.
Den sjuksköterskeledda mottagningen var säker och effektiv för patienterna och tillförde ett mervärde i vården genom att patienterna erfor trygghet, förtrogenhet och delaktighet.

– Det visade sig att personcentrerad vård är kärnan i reumatologisk omvårdnad och leder till en jämbördig relation mellan patient och sjuksköterska. Avhandlingen stöder införandet av personcentrerad vård i reumatologisk omvårdnad och forskning, säger Ingrid Larsson.
Doktorsavhandlingen framläggs till offentlig granskning fredagen den 17 maj 2013 kl. 13.00 i Forum Humanum, Hälsohögskolan i Jönköping. Doktorsavhandlingens titel är ”Person-centred care in rheumatology nursing in patients undergoing biological therapy: An explorative and interventional study”.

Guldregn över FoU Spenshult!

I veckan har Reumatikerförundet meddelat resultatet av årets utdelning av forskningsanslag från Reumatikerfonden. I konkurrens med andra forskare runt om i Sverige har medarbetarna på FoU Spenshult vunnit anslag på sammanlagt 400 000 kr. Detta innebär ett mycket värdefullt tillskott för vår forskning kring sjukdomar i rörelseapparaten. Reumatikerförbundet, som en gång var instiftare till FoU Spenshult, är är även vid sidan av dessa anslag en viktig samarbetspartner i patientnära forskningsprojekt och utveckling av vården för personer med reumatiska sjukdomar.

Här vill vi presentera vår verksamhet och våra resultat, men också sprida kunskap kring sjukdomar i muskler, leder och skelett. Hemsidan är under utveckling så kom gärna tillbaka och upptäck nyheter. Under våren tänker vi lägga ut föreläsningar både i text och som videoinspelningar.

Vi blickar fram mot ett spännande 2013. Under året kommer ett par medarbetare att lägga fram sina avhandlingsarbeten. Sjuksköterskan Ingrid Larsson har studerat betydelsen av mötet mellan patient och sjuksköterska vid avancerad medicinsk behandling av reumatisk sjukdom. Sjukgymnasten Emma Haglund har studerat förekomsten av reumatisk ryggsjukdom och hur denna påverkar fysisk aktivitet och förmågan att vara produktiv på arbetet. Ingrid har planerad disputation den 17 maj och Emma den 8 november.

I samarbete med Reumatikerförbundet utvecklar vi nu projekt där patienterna själva satt agendan för forskningen. Detta skapar nya infallsvinklar på begrepp som hälsa och livskvalitet. Hälsa är också i fokus när vi fördjupar vår samverkan med Högskolan i Halmstad. FoU Spenshult är en viktig samarbetspartner för högskolan när man nu söker forskarutbildningsrättigheter inom ämnet Hälsa och Livsstil.

Under 2013 öppnar vi upp för samverkan med omgivande näringsliv, med en möjlighet att adoptera en forskare. Tanken med detta är att skapa en ömsesidighet i relationen med företag som är intresserade av att stödja forskningen på FoU Spenshult. Även enskilda personer är förstås välkomna att stödja vår verksamhet. Genom åren har vi tagit emot både stora donationer och mindre gåvor. Senast vid årsskiftet fick vi en större donation, som tillsammans med andra bidrag och anslag skapar förutsättningar för nya forskningsprojekt med syftet att ge bättre vård och livskvalitet för personer med sjukdomar i rörelseapparaten.
